Toddler vomiting on a regular basis?

My sister's 4 year old vomits on a regular basis. He will look at you and say I am going to "upchuck". He then will give you enough time to give him something to throw up in.... It is not when he is being punished or stressed. It is at any time of the day or night. It has not been pinpointed to any certain food. When he was a baby he would PROJECTILE vomit at least 1-2 times a day. Their MD said this was normal, while I was screaming "NO WAY"! He is low weight but average height and an INCREDIBLY picky eater. I have encouraged her to get a new MD or even go to a specialist. Does this sound like perhaps he had untreated reflux as a baby and now may have a damaged esophaguss? She is taking him to the MD today and I want your opinion. Thanks!
